PREPERATION WORKSEET FOR RODENT TREATMENT

____    To the best of your ability, please move items away from the walls in basement.
____    Do not remove any evidence of rodent activity. Rodent dropping will aid your
            service technician ability to determine areas to place rodent control equipment.
____    If at all possable, remove any debris from outside home foundation.(ie.overgrown
           weeds, old wood, etc.)
____    A complete inspection inside and outside of your home will be conducted by your
            service technician prior to trap placement. We will seal all rodent entry points that
            are ½’’ or less, excluding gaps under doorways & garage doors. Your service
            technician will point out any areas of the exterior of your home that needs to be
            repaired by a carpenter. Our office has a list of reliable carpenters that have done
            work for us in the past.
____    Because of the many circumstances which remain out of our control concerning
            structural issues with the exterior of your home, we do not offer any warrantees
            for rodent control.
____    After this treatment is preformed, you may want to consider our Quarterly insect
            and rodent control program. Our office staff will give you a courtesy call in about
            one month to see if you would like to begin this program.
____    With the Quarterly program you will be given a full 90day warranty for both insects and rodents.
